Because every family is unique, the Faith and Family Hospice creates individual plans of care to honor the patient’s and family’s wishes for the life that the patient has. We take a collaborative approach to care, informing the patient and family every step along the 360° hospice continuum. Our professional caregivers work to ensure that and caregivers:
- Develop a personalized, thoughtful care plan together with the patient and family
- Manage physical symptoms and keep the patient comfortable
- Provide emotional and spiritual counseling and support for the patient and family
- Help the patient and family members understand changes in the patient’s ongoing medical condition
- Support the patient in daily living activities
- Train family members on skills to help confidently care for loved ones
- Work with a patient’s primary caregivers to provide information about care and address any changes that might occur
- Assist with resources, referrals, and education that might be helpful
- Offer bereavement counseling for loved ones and their families after the death of the patient
